Should women over 50 strength train on a GLP-1?

Yes: for most women over 50 on a prescribed GLP-1, strength training on at least 2 days a week, with enough protein and a check on bone health, is the most useful thing to add. Part of the weight lost on these medicines is lean mass, and the menopause transition already nudges muscle and bone the wrong way. You cannot control the first. You can push back on both.

The National Institute on Aging (2024) puts the average age of menopause at 52 in the United States and says women may lose muscle and gain fat during the transition. A review by Locatelli and colleagues in Diabetes Care (2024) reports lean-mass loss of around 10% with incretin-based drugs. Neither number is a verdict on you. Together they explain why a GLP-1 at 54 deserves a different plan than a GLP-1 at 34.

What changes after 50, and what the list does about it

What the sources sayThe checklist item that answers it
Women may lose muscle and gain fat during menopause (NIA, 2024).Two strength days, five moves, last reps that feel like work.
After menopause, women are more vulnerable to osteoporosis (NIA, 2024).Ask about bone health; keep loading your legs and hips.
Lean mass can fall by around 10% on incretin drugs (Locatelli, 2024).Strength work plus a protein target set with a clinician.
Adults need muscle-strengthening on 2+ days a week (CDC, 2023).Strength booked on the calendar, not squeezed in.

Honest limits

This checklist is general and so is Sturdy. Neither knows your bone density, your blood pressure or your knees. Sturdy does not suggest doses, change medication schedules or give medical conclusions; its chair-stand history has no scores or age norms. If you want a program built around a diagnosed condition, a physical therapist is the better pick, and we would rather say that than pretend otherwise.

How to start in 3 steps

  1. Tonight: tick items 1 and 2. Message your prescriber's office and pick two days.
  2. This week: do your first session with five moves, and take your chair-stand baseline.
  3. In four weeks: repeat the chair stand, look at what you logged, and bring one page to your next appointment.

A mini story: the grocery bags

Fifty-four, four months on a GLP-1, eighteen pounds down. The scale is happy. The grocery bags are not: the walk from the car feels longer than it did in spring, and the stairs with a laundry basket now come with a pause on the landing.

She does not join a gym. She picks Tuesday and Friday, pushes the dining chair against the wall and does five moves for twenty minutes: sit-to-stands, a counter push-up, a band row, a hinge with a backpack, and a carry with the actual grocery bags. In week one she manages 9 chair stands in 30 seconds. She writes it down. At her next appointment she asks one question about protein and one about bones, and leaves with answers instead of a vague plan to “do more.”

Questions women ask

Should women over 50 strength train on a GLP-1?

Strength training is widely recommended for adults, and it matters more when weight is dropping. A review by Locatelli and colleagues in Diabetes Care (2024) reports lean-mass loss of around 10% with incretin-based drugs and proposes resistance exercise alongside them. Ask your clinician what is safe for you.

Does menopause make muscle loss on GLP-1 worse?

Menopause brings its own changes. The National Institute on Aging (2024) says women may lose muscle and gain fat during the transition, and are more vulnerable to osteoporosis afterwards. That overlap is why this checklist pairs strength with bone and protein questions.

How many days a week should I lift?

Start with 2, as CDC (2023) recommends for adults, with a rest day between. A third day is a fine goal once two feel routine.

Do I need a gym?

No. Body weight, a resistance band and a pair of dumbbells cover all five movement patterns at home. A gym helps later if you want heavier loads.

What if I have joint pain?

Choose the version of each move that does not hurt (a higher chair, a counter push-up instead of the floor) and tell your clinician. Pain that is sharp or lasts past the next day is a reason to stop and ask.

Is Sturdy free?

Weekly planning, all three workout types, the core movement library, sessions up to 30 minutes, protein logging, medication records, check-ins and 30-day summaries are free. Sturdy Pro adds gym-equipment movements, 45-minute sessions, label scanning and longer progress views.
